We are the only skilled nursing facility in Douglas County. Many families come to us for short-term rehabilitation suites after surgery, stroke recovery, or orthopedic care. Others need long-term skilled nursing or specialized pulmonary and ventilator care. No matter the length of stay, advance planning helps the transition go smoothly.
What to Bring on Admission Day
Packing the right items on day one saves time and keeps your loved one comfortable. Gather these basics before arrival:
- Government-issued photo ID and insurance cards (Medicare, Medicaid, or private coverage)
- List of current medications, dosages, and prescribing physicians
- Advance directive or power of attorney documents, if available
- Comfortable clothing for several days, labeled with the resident's name
- Non-skid footwear and personal toiletries
- Eyeglasses, hearing aids, and labeled dentures with storage cases
- Cell phone and charger, if the resident uses one in the room
- Contact information for the primary care doctor and hospital discharge planner

What Happens During Your Stay
After admission, our clinical team completes an assessment and builds an individualized care plan. For rehabilitation patients, licensed therapists develop a schedule that may include physical, occupational, and speech therapy up to six days per week. Nursing staff monitor medications, wound care, IV therapy, and other medical needs around the clock.
Family members stay involved through care conferences and discharge planning meetings. Social services staff help coordinate equipment, follow-up appointments, and home support when the resident is ready to leave. Questions Worth Asking Before You Choose
Comparing facilities takes more than a brochure. Ask about nurse staffing ratios, therapy frequency, physician coverage, and how discharge planning begins. Tour the campus, meet the administrator, and ask to see the therapy gym and resident rooms.
