After admission, our clinical team completes an assessment and builds an individualized care plan. For rehabilitation patients, licensed therapists develop a schedule that may include physical, occupational, and speech therapy up to six days per week. Nursing staff monitor medications, wound care, IV therapy, and other medical needs around the clock.
Family members stay involved through care conferences and discharge planning meetings. Social services staff help coordinate equipment, follow-up appointments, and home support when the resident is ready to leave.
